The RIVER trial pre-specified analysis presented at ESC 2022 aimed to test the efficacy/safety of rivaroxaban compared to warfarin in patients with atrial fibrillation (AF) and bioprosthetic mitral valves, according to time of valve implantation. In this touchCARDIO interview, we speak with Professor Renato Lopes (Duke University Medical Center, Durham, NC, USA) to discuss non-vitamin K antagonist oral anticoagulants (NOACs), also known as direct oral anticoagulants (DOACs), in patients with AF and bioprosthetic valves as well as the RIVER trial and what questions remain unanswered that require further studies. The abstract entitled ‘RIVER: Rivaroxaban for Valvular Heart Disease and Atrial Fibrillation’ was presented at the European Society of Cardiology (ESC) Congress 2022, 26 – 29 August 2022. 60 year-old male presents with worsening exertional dyspnea (NYHA Class III) and multiple admissions for CHF exacerbation in past 3 months. Past medical history is significant for symptomatic severe aortic insufficiency s/p bioprosthetic AVR (2017) with 23mm Magna 3000 valve, ESRD on HD, HTN and HLD. Recent echocardiogram revealed bioprosthetic valve degeneration with severe aortic stenosis (PG/MG/AVA of 75/51/0.67) and depressed LVEF of 30%. Coronary angiogram showed moderate 2V CAD being medically managed. Lower extremities on CT angiogram revealed calcified arteries with vessel size < 6mm. The Internal diameter of surgical AV measured 21.5×22.3mm (mean 21.9mm), perimeter 68.3mm and the area 370.6 mm2. The STS mortality risk for surgical AVR was calculated at 5.83%. The patient underwent Heart Team evaluation and was found to be at high risk for re-do SAVR due to comorbidities and frailty. Now planned for Valve-in-Valve TAVR with 26mm Evolut Pro+ CoreValve and bioprosthetic valve fracture using 23mm True Balloon via right percutaneous femoral approach.

At the 2010 Oxford Medtronic lecture, over 120 academics, students and representatives from industry and government came to the Richard Doll Centre in Headington, Oxford, to hear Professor Mark Prausnitz, PhD talk about "biophysical methods of drug delivery". Professor Prausnitz is Professor of Chemical and Biomedical Engineering and Director of the Centre for Drug Design, Development and Delivery at the Georgia Institute of Technology. Professor Prausnitz delivered an eloquent lecture illustrating the use of biomedical engineering techniques to enhance intracellular and transdermal drug delivery. In particular, he demonstrated the use of ultrasound to increase cell membrane permeability reversibly, and the exploitation of localised heating, abrasion and micro-needle technologies for improved transdermal drug and vaccine delivery. The Regional Vice President of Medtronic for UK & Ireland, Mr Geoff Morris, OBE, highlighted the UK’s strong scientific base which provides an effective and successful platform for clinical research. He said: "Committing to the future of scientific research through graduate scholarships is crucial if the UK is going to make a significant contribution to global scientific research and the management of chronic disease."
